Reporting Injuries
It is important that all injuries, incidents, or near misses that take place
on campus (and in the field) are reported to Human Resources. Incident
report forms help to identify areas that need strengthening by addressing
the root cause and identifying training/procedural needs to prevent such
injuries from recurring. Incidents can involve students, staff, faculty,
visitors, or contractors. Reporting can include a wide variety of incidents
such as injuries from slips or falls, animal bites, chemical or biological
spills/exposures, needle-stick injuries, back injuries from lifting,
carpal tunnel syndrome, etc.
For further information on incident reporting and to access the appropriate
forms, please visit
The policy on incident reporting and worker's compensation requirements is
located on the policy website at:
If you have any questions, please contact the 秀色短视频 health and safety
